- Record the head a CI wait measures in a new ci_wait_sha card field
- A head the card did not record starts a fresh wait: both the appearance budget and the timeout restart
- phase-merge re-reads the wait start on each retry
- Regression tests for resumed phase-pr, phase-merge recovery re-entry and the gate's first read
docs(IW-491): record the pre-push SSH idle timeout follow-up
