- SessionHooks.runSetups and HookOps.runSessionSetupHooks run the setup hooks only, never a SessionAction
- iw start --no-agent creates the worktree and session, runs setups, launches no agent and does not attach
- The combination of --no-agent and --prompt is refused before any side effect
- Harness, unit and BATS tests; README and docs/api updated
