An effort slug in --issue-id now always takes the slice card; an unreadable
pointer keeps its reason; ci-gate parses --issue-id without --input.

- ReviewStateLocation.cardLocations routes on the key kind, slice and a SlicePointer value; the cwd is no longer an input
- write, update, path and ci-gate all locate the card through it; an unreadable pointer reports its reason
- path subcommand moved to ReviewStatePathCommand; keyFromFlag moved to WorktreeKeyResolver.fromFlagOrBranch
- Harness, unit and BATS tests; API docs, release notes and follow-ups
