- Add ProcessAdapter.NoTimeout and require timeoutMs in streamOrExplain
- tmux and herdr attachSession run with NoTimeout; switchSession keeps DefaultTimeoutMs
- Unit tests plus IW_SLOW-gated integration and E2E tests, one local run each green
- docs/api/Process.md updated; review findings recorded in FOLLOW-UPS.md
