Adds a Superseded marker at the eight places that describe the
state-only merge path or ci_gated_sha as current, and records the stale
CommitPolicy and --no-wait passages in FOLLOW-UPS.md.

- Departure 59 records that the card leaves the tree and both merging commands gate the head they merge
- Superseded markers on every ci_gated_sha and state-only passage
- FOLLOW-UPS lists the stale CommitPolicy and no-wait passages
